Ask what this professional can help with, how notes are kept, who can see them and what exceptions to confidentiality apply in that service. These are questions to clarify locally, not promises of absolute secrecy. You can first explain that smoking is changing and ask how the visit will work.

Ask how observations will be assessed

“What should I tell you if sleep, mood or daily functioning changes?” is more useful than deciding that a change must be withdrawal. If your professional and prescriber are different, ask who will communicate the relevant information and with what consent. Immediate danger needs the current local emergency route, not an unread message or the next appointment.
