Write a useful observation, not a causal label

Fictional edit: replace “withdrawal made me dizzy” with “I felt unsteady while standing; I had stopped smoking earlier.” The second version separates sensation, context and chronology. Neither sentence establishes a diagnosis. A short factual note can support the clinical conversation without proving why the change happened.

Include what a professional needs to check

Note the change, its beginning and course, its effect on an ordinary activity and the question you want answered. If relevant, bring actual medicine or product information and what was used; do not change it to make the note fit a theory. There is no requirement to document every episode or complete a template before requesting help.
